I denne artikel beskrives konceptet ’Den digitale patientmappe’, som er udviklet som en del af ELYK projektet – E-læring, Yderkanter og Klyngedannelse. Den digitale patientmappe er resultatet af et co-designforløb mellem privatpraktiserende fysioterapeuter og medarbejdere i ELYK, der har udmøntet sig i et værktøj, der muliggør et tæt kommunikativt samarbejde mellem fysioterapeut og patient. Registreringer, diagnose samt instruktioner samles digitalt ved hjælp af tekst, billede, lyd og video og gøres tilgængeligt for patient og terapeut i alle relevante situationer. Den digitale patientmappe kan ses som en udvidelse af patientjournalen med den forskel, at patientmappen er patientens, og at den indgår i et mere ligeværdigt samarbejdsforhold mellem patient og terapeut.
