En oversigt over eksistentielle bekymringer hos hjertestopoverlevere og implikationerne for rehabilitering: Har vi brug for et nyt værktøj i værktøjskassen?

Resumé

Mange mennesker som overlever et hjertestop vil lide af langsigtede sekundære fysiske og følelsesmæssige konsekvenser som følge af hjertestoppet. Dokumentation, overvejende fra kvalitative undersøgelser, indikerer, at eksistentielle bekymringer, herunder frygten for at dø, frygt for nyt hjertestop og mangel på mening, også kan betragtes som en vigtig sekundær konsekvens. Internationale retningslinjer anbefaler rehabilitering efter hjertestop, som har sekundær forebyggelse af hjertesygdomme til formål. Indtil nu har modellerne for rehabilitering dog ikke eksplicit omfattet eksistentielle eller åndelige bekymringer, selvom disse er del af ”total pain”-modellen, som anvendes inden for palliativ pleje. Eksistentielle bekymringer kan have en negativ indvirkning på rehabiliteringen grundet frygt for at bevæge sig/kinesiofobi eller tab af mening, hvilket fører til nedsat deltagelse. Omvendt rapporterer nogle overlevende, at deres hjertestop har givet dem en ny chance i livet, en vilje til at forblive raske, hvilket potentielt øger deres deltagelse i rehabilitering. At undersøge hjertestopoverlevere ift. eksistentielle problemstillinger kunne bidrage til, at identificere de personer, som ville have gavn af interventioner, som har til formål at mindske disse bekymringer. Derudover kunne man potentielt identificere overlevere, for hvem eksistentielle bekymringer udgør en barriere for, at deltage i rehabilitering. Der er imidlertid behov for mere forskning i, hvordan man undersøger hjertestopoverlevere for eksistentielle bekymringer, hvordan disse bekymringer påvirker deltagelse i rehabilitering, samt for at identificere effektive metoder til at afhjælpe bekymringerne for at sikre, at overlevende efter hjertestop kan vende tilbage til det bedst mulige liv.
